  Requires Adobe Reader v7.0 to fill out forms online. It can be downloaded for free at the link below
  

There are now two ways to fill out the DARRT4 Membership application.

1. You can fill out the forms online and print and sign. Them submit to your local EM director.
You need the newest version (7.0) of Adobe Reader (free) to fill out the forms online.

2. Print the application form and fill out by hand then submit to your local EM director
